School or District Reporting Form

In accordance with section 1012.796, Florida Statutes, each school district, charter school and private school that accepts scholarship students under section 220.187 and 1002.39, Florida Statutes, shall file in writing with the department all legally sufficient complaints within 30 days after the date on which the complaint comes to the attention of the school.

The attached form should be used by public schools or districts, charter schools and private schools to report legally sufficient allegations of misconduct by certified educators to the Office of Professional Practices Services and should not be altered or modified by the user. The form should be completed in its entirety and submitted with all supporting materials.
